Overview
Registered Nurse – Home Health in Bala Cynwyd, PA, travel temporary assignment starting 04/15/2026 for 12 weeks, with guaranteed 40 hours per week. Shift: 8:00 AM–4:30 PM (5x8). Setting: Home Healthcare. Weekly pay range: $2,293–$2,485 per week.

Responsibilities
- Provide RN-level clinical care to patients in a home setting, including assessment and ongoing monitoring.
- Manage case management duties, coordinating with the patient’s interdisciplinary team to develop and adjust care plans.
- Complete and document OASIS assessments as part of standard home health documentation.
- Maintain a high-volume caseload, typically caring for 5–6 patients per day with accurate and timely documentation.
- Perform wound care when indicated and preferred, following best-practice protocols.
- Educate patients and caregivers on disease management, safety, medications, and self-care to promote outcomes and prevent hospital readmissions.
- Ensure visits and documentation are completed in a timely manner and in compliance with regulations and agency policies.

Requirements
- Active RN license in Pennsylvania
- Minimum 2 years of home health experience
- Case management experience and proficiency with OASIS documentation
- Wound care experience preferred
- Ability to manage a busy home health caseload and work autonomously in patients’ homes
- Willingness to travel within the assigned service area and to work a full 40-hour week
